Augmentative Communication Evaluations

CATIES conducts augmentative communication evaluations on individuals with speech/language difficulties to determine methods to improve communication.

If you are interested in scheduling an evaluation, please complete a request form using the link in Step 1 below. Please review the guidelines for requesting an augmentative communication evaluation for additional information.

 

Procedure for Requesting an Augmentative Communication Evaluation Step 1: If you are unable to access the online form, please complete the downloadable version. Augmentative Communication Evaluation Request Form (online) Augmentative Communication Evaluation Request Form (downloadable) Step 2: A CATIES staff member will review your request and contact you to finalize plans, discuss the fee schedule and review evaluation guidelines.   Step 3: Please download and complete the appropriate forms: School representative (AC) *required Parent/Guardian (AC) *required Physical therapist (AC) Occupational therapist (AC) Speech therapist (AC)

 

We request that the individual scheduling the evaluation is responsible for ensuring that all forms are completed and returned 10 business days prior to the evaluation.

An augmentative communication evaluation includes:

A review of the student's educational history as provided by parents, teachers, therapists and school district personnel

An individualized evaluation by a speech/language pathologist.   This most often occurs at CATIES at The College of New Jersey but can be scheduled at other locations under unique circumstances and at additional cost.

A written report with recommendations for augmentative communication implementation. The report provides specific product information in order to assist the school district, agency or parent in implementing recommendations expeditiously.
